2. Family Division Confidential Asset and Liability Disclosure Sheet

3. Facilitation Order.

5. Indigent Appointed Attorneys' billing form
This is the form attorneys who are appointed to represent indigent criminal defendants in our Court are to submitted Judicial Aide for payment. Use of the form greatly increases the efficiency of bill processing. We recommend that counsel photocopy a copy, keep in it their file and fill in the services they perform as each case proceeds.
